To get started, DataCheck will need to know the types of background investigations you would like to order. You may click on Request a Price Quote above, or call us at 800-253-3394 and ask to speak to a sales representative. Once we have an idea of the background checks you would like to order, we will email you a price quote. Once you are ready to get started, we will email you our Quick Start Kit and the Service Agreement and Notification of Terms to you.

Our Quick Start Kit includes the following forms:

• Background Check Order Form - when ordering background check requests, this form lets us know the types of checks you would like to order.
• Consumer Authorization & Disclosure Form - all applicants must be notified in writing that a background check (consumer investigative report) will be performed on them.
• Background Check Release Form - all background checks are done with the applicant's permission and complete knowledge.
• Summary of Rights Under the Fair Credit Reporting Act - before ordering a background check, you must provide the applicant with a copy of their rights under the Fair Credit Reporting Act.
• Sample Decline Letter - the Fair Credit Reporting Act states that you must send the applicant a written letter with DataCheck's name and address should you decline their application for the job based in whole or in part on our background investigation.

Our Service Agreement and Notification of Terms
This agreement provides information as to our terms, and the responsibilities of DataCheck and your company as it pertains to Consumer Investigative Reports and abiding by the Fair Credit Reporting Act. This Agreement is not a contract to use our services exclusively. You are under no obligation to use our services by signing this agreement. DataCheck wants you to use us as your background screening provider because you are pleased with our services.
