Our Quick Start Kit provides you with all the necessary forms to order background checks:

• Background Check Order Form - when faxing background check requests, this form lets us know the types of checks you would like to order.
• Consumer Authorization & Disclosure Form - all applicants must be notified in writing that a background check (consumer investigative report) will be performed on them.
• Background Check Release Form - all background checks are done with the applicant's permission and complete knowledge.
• Summary of Rights Under the Fair Credit Reporting Act - before ordering a background check, you must provide the applicant with a copy of their rights under the Fair Credit Reporting Act.
• Sample Decline Letter - the Fair Credit Reporting Act states that you must send the applicant a written letter with DataCheck's name and address should you decline their application for the job based in whole or in part on our background investigation.

Prior to using our services, you must complete, sign, and fax the Service Agreement and Notification of Terms to us (see the Getting Started page). Once we receive this form, we will provide you with your Quick Start Kit.
